A number of forms are used by the Office of Alumni Affairs throughout its web pages. A alphabetical list with a link to the document is provided here.

Nominating an alumnus/alumna for either Outstanding Alumni Ambassadors or

Distinguished Alumni requiers a submission using the Achiever Nomination Form.  Any materials that a nominating person wishes to attach can be sent in with the form;all materials will become the property of WNMU.

Other WNMU departments may print off and copy the Alumni Contact form

when they are at other functions to capture WNMU alumni data.  Please use the following form and returned filled out forms to Alumni Affairs for updating the Alumni database.

Alumni and friends may print off, fill out, and return to Alumni Affairs for processing theirparticipation in this program.

Great Race attendees may print off, fill out, and return to Alumni Affairs for

processing and preparing their attendance materials for Great Race.

Homecoming attendees may print off, fill out, and return to Alumni Affairs for

processing and preparing their homecoming packets for Homecoming.

WNMU constituencies can use the following form to schedule a Sodexo

dessert request through Alumni Affairs for a student's birthday or other staff /

faculty function.  At least ONE WEEK prior to date needed is required to

schedule this.

Alumni or other WNMU constituencies can use the following form to request

a plaque for WNMU alumni who qualify for display in the Brancheau PE
