Staff Benefits Summary-Fiscal Year 2001-2002

  1. Basic Life Insurance - $50,000 Employee Only, Prudential, no cost to the employee

    2. Primary Care Physician (PCP)-Coordinated Network; (no deductible); $20 office visit co-pay

    3. CIGNA/Lovelace Dual Option Plan (must select primary care physician); $15 office visit co-pay
    4. Cimarron Regional HMO (must select primary care physician); $15 office visit co-pay
  1. Dental Care United Concordia; $50 deductible, no charge for basic services. Choice of High option or Low option. (See * below)
  2. Vision Care Davis Vision: $10 deductible examination, $15 deductible materials. (See * below)
  3. Long Term Disability Prudential: 30 day waiting period, pays 66 2/3% of lost wages after annual/sick leave balances are exhausted.
  4. Voluntary Life Insurance Prudential Voluntary Life and AD&D: Premium paid 100% by employee-Available coverage 1x, 2x, 3x annual salary, spouse up to 1x annual salary, dependent $5,000.
  5. IRS Flexible Benefit Plan Premiums deducted on a pre-tax basis thereby reducing taxable income and increasing net pay.
  6. Annual Leave Full time employees can earn eight (8) hours per month = ninety-six (96) hours per year (maximum accrued is 160 hours = 4 weeks); not eligible for use during trial period. Leave does accrue during trial period.
  7. Sick Leave Full time employees can earn twelve (12) hours per month = 132 hours per year (maximum accrued 1040 hours = 26 weeks); not eligible for use during trial period. Leave does accrue during trial period.
  8. Holidays Eleven (11) paid holidays per year.
  9. Educational Assistance Program Tuition Waiver: 12 credit hours per year beginning with Summer I and ending with the Spring semester (SSI, SSII, FA, SP). Waiver amount $1021.25 for the Bachelor’s program and $1093.25 for the Graduate Program. Tuition waivers are available for the employee, spouse and/or dependents (no waivers will be approved while the employee is in the trial period and proof of dependents may be required by the Human Resources Department).
  10. Retirement Plan Educational Retirement Plan: Employees contribute 7.6 %, employer contribute 8.65 %.
