VACATION AND SICK LEAVE POLICY
EXEMPT AND NON-EXEMPT STAFF
Employees wishing to take vacation or sick leave should complete a Leave Request Form. This form should be signed by the employee and approved by the employee’s supervisor.
The Leave Request Form is a three part form. The white copy should be forwarded to Human Resources in accordance with the University’s Attendance Policy. The yellow copy is retained in the department’s file, and the pink copy is kept by the employee. Leave Request Forms for sick leave should be kept in a separate confidential file in Human Resources Office.
Vacation and sick leave balances will be reported to employees semi-annually. The Human Resources Department will be maintain the “official” leave balance. Employees wishing to contest their balance may contact the Human Resources Department on an individual basis and arrange a meeting with the Director (or designee) at a mutually agreed upon time to inspect their attendance files.
